How to Get Proof of Underlying Medical Condition

Individuals with specific documented medical conditions are eligible for a COVID-19 vaccine. Proof of condition may be required depending on where you schedule your appointment.

First, follow these steps to determine your eligibility and access your medical records for proof of condition.

Then, to schedule an appointment, use the New York State (NYS) “Am I eligible?” online tool, call the NYS Vaccination hotline at 1-833-697-4829, or visit CVS or Walgreens or your county’s health department website. Please note Nuvance Health is not administering vaccines at our hospitals or practices.

  1. Determine if you are eligible by reviewing the NYS list of medical conditions. The conditions are also listed below.
  2. If you are eligible, find proof of your condition(s) in your medical record.
  3. To access your medical record, log in to your MyHQ247 patient portal:
  4. When you are logged in, select the Health Record icon:
    • If you are on a computer, the Health Record icon is in the upper left corner.
    • If you are on a mobile device, select Menu in the upper left corner, then Health Record from the menu options.
  5. Select Health Profile.
  6. Look for the Health Issues section on the Health Profile to see your medical conditions:
    • If you are on a computer, select the Print icon in the upper right corner. Check Health Issues under Print Options and then the Print button.
    • If you are on a mobile device, take a screenshot and save it to your Photo Library. How to take a screenshot varies depending on the device.

Follow these steps if a doctor’s note is required where you schedule your appointment:

  1. When you are logged in, select the Messaging icon:
    • If you are on a computer, the Health Record icon is in the upper left corner.
    • If you are on a mobile device, select Menu in the upper left corner, then Messaging from the menu options.
  2. Select the Send a Message button.
  3. Search for your physician’s name in the To field.
  4. In the Subject field, put COVID-19 health conditions doctor’s note.
  5. In the Message field, put that you are eligible for a vaccine and need proof of medical condition(s).

Individuals with any of the following medical conditions are eligible for a vaccine

  • Cancer (current or in remission, including 9/11-related cancers)
  • Chronic kidney disease
  • Pulmonary disease, including but not limited to COPD (chronic obstructive pulmonary disease), asthma (moderate-to-severe), pulmonary fibrosis, cystic fibrosis and 9/11-related pulmonary diseases
  • Intellectual and developmental disabilities including Down syndrome
  • Heart conditions, including but not limited to heart failure, coronary artery disease, cardiomyopathies or hypertension (high blood pressure)
  • Immunocompromised state (weakened immune system), including but not limited to solid organ transplant or from blood or bone marrow transplant, immune deficiencies, HIV, use of corticosteroids, use of other immune weakening medicines or other causes
  • Severe obesity (body mass index of 40 kg/m2 or higher), obesity (body mass index of between 30 kg/m2 and 40 kg/m2)
  • Pregnancy
  • Sickle cell disease or thalassemia
  • Type 1 or 2 diabetes mellitus
  • Cerebrovascular disease (affects blood vessels and blood supply to the brain)
  • Neurologic conditions including but not limited to Alzheimer’s disease or dementia
  • Liver disease
