The skilled primary care teams at Nuvance Health understand geriatric care and the nuances of aging. Your primary care provider (PCP) is your first stop for answers to aging questions, concerns or challenges. Older adults are strongly encouraged to have an Annual Wellness Visit. While not a head-to-toe physical exam, this one-on-one discussion with your provider is covered by Medicare and includes:
Medical and family history review
- Focus on your overall health and preventive care
- Discussion about your medications and specialty care
- Routine measurements of height, weight and body mass index
- Environmental safety, depression and dementia screening
- Advanced care planning

Center for Healthy Aging
You or loved ones may receive care for a range of geriatric and aging issues at The Center for Healthy Aging, located on the campus of Northern Dutchess Hospital in Rhinebeck, NY. Specialists from a number of medical specialties help those who are eligible manage issues including memory loss, gait and balance, medication management and more.
Eligible individuals include adults 65 and older who have experienced falls and a change or decline in their physical mobility. They also include patients with cognitive disorders and/or memory loss, including Alzheimer’s disease.
The center’s medical director is a family practice physician with fellowship training in geriatrics. A licensed social worker specializes in geriatrics and caregiver support, offering you experience and compassionate care.
Adults 65 and older who are hospitalized or at risk for acute fractures (broken bones) receive personal attention based on their condition. The skilled team also evaluates bone health for signs of osteoporosis (bone weakening).
Behavioral and Mental Health
Mood and memory changes can occur as you age. The specialized care teams at Nuvance Health are specially trained to address these challenges. You receive diagnostic assessment and treatment that includes your primary care provider and community resources as needed.
You receive compassionate, customized services that help you stay as independent as possible and pursue the activities you enjoy. The team prioritizes your personal goals and health while working to relieve symptoms from issues such as depression and dementia.
