Our Diabetes Self-Management Education program at Putnam Hospital offers counseling specific to diabetes-related nutrition plus other services to support your good health. To help improve health outcomes, the American Diabetes Association recommends utilizing recognized programs like ours at four critical times*:
With a new diagnosis of diabetes
Annually for health maintenance and prevention of complications
When new complicating factors influence self-management
When transitions in care occur
Ask your doctor for a referral to visit us.
We are the Certified Diabetes Care and Education Specialists at Putnam Hospital in Carmel, New York. We tailor each DSMES/T visit to you and your individual self-care needs. Topics may include:
Nutrition
Exercise
Medications
Blood sugar management
Healthy coping
Problem-solving
Reducing your risk of complications

Facilitated by a Certified Diabetes Care and Education Specialist, these meetings offer the opportunity for people living with diabetes to share concerns and to help manage the psychosocial issues associated with diabetes. Occasionally a guest speaker will attend and discuss topics related to diabetes.
