The Information We Collect

At your request, you may access our BeWell patient portal to access information in our medical records about you. Our BeWell patient portal is subject to its own Terms of Use and Privacy Policy, and you should review that Terms of Use and Privacy Policy before accessing and using the BeWell patient portal.
